CSL Mini-Grant Criteria and Proposal Structure
All projects must:
- Involve collaboration between the university and one or more community organizations in the project design and plan; each project must have a faculty mentor.
- Clearly state how the project reflects an approach to service-learning;
- Have a realistic workflow, timeframe and budget; and
- Culminate in a quarterly written report of number of hours logged in community service, number of hours spent in mentorship activities, planning, and reflection.
- Be presented in Spring 2009 at the Campus-Wide Day of Service Learning.
PROPOSAL STRUCTURE
Please use the following guidelines when submitting your 2 page narrative:
- Project description addressing one of the three components of the MSM Model should include: description of community need, documentation of health disparity project will address, projected number of students and community beneficiaries, community partner(s), measurable objectives and anticipated outcomes.
- Budget including project expenses and matching funds (complete attached 'Budget form').
- Names and contact information for faculty who will be involved in the project.
This RFP will be a Closed Application Cycle and funding will be available on a first-come, first- served, campus-wide basis.�� A representative committee with representation across the Health Sciences Center has been convened to provide objective review of the proposals and to ensure that the proposals meet requirement criteria for funding.
